The Progressive Pligmologist: Case Study January 2026

This patient is a 74 year old male with stroke resulting in dense right sided hemiplegia that presented with an ulcer of the right leg. The site had started as a laceration sustained from his motorized wheelchair, and rapidly expanded. Serial debridement of the location eventually led to exposure of complete subdermal failure and hematoma accumulation within the tissue layers. 

Unfortunately despite evacuation of the hematoma and cleaning, this site of breakdown demonstrated no improvement, and he required admission for urgent vascular evaluation, with a subsequent angioplasty performed during his hospitalization. The revascularization dramatically accelerated his response, and hydrofera blue classic with collagen and compression led to rapid closure. This case emphasizes that despite an optimized management approach, ABIs and arterial ultrasound are a critical component to any limb salvage approach.
